Acceptable Methods of Payment
We accept cash, checks, debit cards, VISA & Mastercard. Returned checks will have an additional $30 fee added. Co-payment is due at the time of visit. Arrangements will be made for a payment plan when needed.
We can now accept online bill payments.
Copies of Medical Records are released upon properly signed notification from our patients. A $25 fee per record is charged to cover the administrative costs of copying the record. Legal release requests will include an addtional fee.
Please inform the office as early as possible when your personal billing and patient information changes (i.e. change in address, phone, name, etc.)
